ProAct’s mission is to identify and successfully treat undiagnosed OSA patients and to provide a treatment alternative for the many that are not compliant with prior treatment efforts.

85% of those with OSA are Undiagnosed.

To accomplish this, we are going to challenge the paradigms that have created an environment in which 85% of the patients with OSA are undiagnosed and untreated, and 97% of the current treatments fail to be tolerated over 50% of the time, resulting in a very high treatment dropout rate.

97% of Current Treatments Fail 50% of the Time.

Within a clinically supported, protocol driven, best practices mode of care, sleep testing should transition from 97% lab based ($2,000-5,000) to 95% home tested ($600-700); and treatment should change from 97% CPAP, (not tolerated 50% of the time), to 70% Oral Appliance Therapy, (tolerated over 90% of the time).

Challenge the Paradigms

The payor implications of this picture are significant:

  • Clearly, selecting an overpriced test and an inappropriate mode of treatment that fails 50% of the time is a waste of money.  In addition, the fact that most patients are not managed into an alternative treatment, allowing both the OSA and its comorbid disease to progress unchecked, makes it even worse.
  • Treating OSA dramatically improves employee health and well-being, is effective preventative care, and is also sound economics. It costs less than $4,000 to diagnose and treat OSA patients; but that investment can mitigate hundreds of thousands in costs associated with the impact of OSA on associated diseases, accidents, and absenteeism. Treatment is proven to quickly reduce hospital days by up to 64%, physician expenses by 50% and all other healthcare expenses by up to 45%.
  • It is well documented that OSA has many comorbidities, including diabetes, obesity, hypertension, stroke, and heart disease. Treating OSA can help prevent these conditions, or in patients who already have one or more of these comorbidities, it can mitigate the impact and improve the outcome for the primary diagnosis.

Cost of disease management in West Palm Beach Fl

Within ProAct’s integrated delivery solution, over 95% of patients will be tested in the convenience of their own bed by a Home Sleep Test administered by ProAct; and the diagnosis and recommended course of treatment is determined by a Certified Sleep Specialist.  Approximately 70% of patients, who have mild to moderate OSA (AHI of 5-29) with no extenuating factors, are typically prescribed an Oral Appliance (OA), which is tolerated by over 90% of patients. OAs are fitted by a ProAct dentist who is trained in oral appliance therapy and credentialed by the accredited ProAct Provider Health Network.

For the 30% who have severe OSA, ProAct’s DME Division supplies a Positive Airway Pressure (PAP) device and manages compliance. If the patient proves to be intolerant, they are referred to a ProAct Provider Health Network dentist to receive a OA, rather than drop out of care.

The ProAct Solution

ProAct brings a next generation disease management platform to this fragmented environment, managing the full continuum of OSA care to deliver a high value proposition to patients, medical providers and payors. ProAct delivers the “full continuum” of care for OSA using the cloud-based iSync® System that creates a virtual delivery infrastructure linking all providers delivering services. The iSync® System maximizes interdisciplinary communication, protocol driven care, and outcomes orientation.

By linking ProAct’s Testing and Diagnosis Division, DME Division, and the dentists in the Provider Health Network Division with referring PCPs, ENTs, Bariatric physicians, Cardiologists, or any specialists interested a total solution for their OSA patients, ProAct offers the solution by managing the total continuum of care: screening, testing, diagnosis, treatment, and ongoing patient management.

A Virtual Delivery Infrastructure

Chronic Disease Management in Kissimmee FL

The iSync® System represents the future of people/patient management with innovative clinical protocols, process flows, integrated patient care, and sophisticated automated patient communications. The platform empowers providers and consumers to leverage information gathered from healthcare apps to better manage the disease, improving the outcomes and quality of care, while reducing costs.

Use of the iSync® System is Critical to Success

To successfully challenge the epidemic of undiagnosed OSA, payors, providers and wellness programs must screen the patient populations under their influence to identify those in need of care; they will find that over 20% will have OSA. The iSync® System starts with a convenient and effective tool for assessing patients and entering patient data into the medical record, the first step in the continuum of care. ProAct offers an exceptional, no cost screening tool to participating physicians; the Assessment can be completed in less than five minutes in the waiting room, digitally scored, and made available to you to assist the patient in finding care. Participating providers can eScript Home Sleep Testing and Diagnosis or treatment through their iSync® portal.

The iSync® System is designed to:

  • Meet the complex day-to-day needs of the ProAct Provider Health Network dentist to deliver care rendered in a dental office within medical insurance reimbursement parameters;
  • Meet the needs of the referring physician to easily screen and refer patients, while continuing to stay in the management loop during treatment; and
  • Link all participants to maximize the continuity of care and treatment outcomes.
